Patient Name: 92 Year Old Female

Discharge Date: 8/16/23

Discharge Destination: Home

Initial Condition:

Patient a 92 y/o female, was admitted to Brighton House after a fall. She was unable to transfer independently and required assistance with moving from one place to another due to mobility issues.

Treatment and Progress:

During her stay, patient received medical care and rehabilitation services to address her mobility issues. During this time, she received physical therapy and Occupational Therapy assistance with ADLs to regain her functional abilities.

Outcome at Discharge:

By the time of her discharge on August 16th, patient had made significant progress. She was able to ambulate short distances independently, transfer, and eat independently.

Discharge Plan:

The patient was discharged to her home environment with family support, She was able to continue to receive care and assistance at home to further aid her recovery and maintain her independence.
