Position Name: Social Worker
Location: Harbor House - BaneSouth


Harbor House in Hingham, MA is seeking a Social Worker (LSW or LCSW) to join our fantastic team.

This is an exciting opportunity to work at this highly sought-after skilled nursing facility and work with an amazing leadership team and great staff.

Harbor House is a 5 Star CMS rated, 138 bed, premier skilled nursing and short-term rehabilitation facility. Supportive team environment in a beautiful building near the Harbor.

We are seeking an LSW or LCSW (required) for Short-Term rehab patients and Long-Term Care residents. Candidate must have excellent assessment and communication skills. Past experience in LTC setting preferred.

Qualifications for the Position

Must possess a minimum of a B.A in Social Worker, LSW or equivalent degree with a current social work license.

Experience

Health Care experience preferred.

Specialized Skills:

Interviewing skills, excellent telephone and communication systems skills; knowledge of community resources; good verbal and written skills.

Essential Personality Traits:

Ability to relate to all levels of staff, ongoing manner towards staff, residents, families and visitors. Ability and willingness to work well with staff and to function as part of the team.

Physical and Environmental Demands:

  • Ability to speak, read and write effectively in English.
  • Work sitting at a desk of extended periods of time with intermittent walling throughout the center. Repetitive finger motion necessary for typing.
  • Exposure to blood borne pathogens as indicated by the OSHA guidelines and does involve occupational exposure to blood or other potentially infectious materials.

Essential Functions of Position:

  • Complies social history data.
  • Participates in assessments, development and evolution of residents care plan.
  • Responsible for planning and leadership interdisciplinary care planning weekly meetings.
  • Documents timely progress notes according to state and federal regulations.
  • Coordinates the discharge planning for residents.
  • Able to interpret and implement established social service policies, procedures and regulations.
  • Coordinates services of community agencies for continued delivery of care upon discharge.
