Ledgewood is a state-of-the-art 123-bed, Medicare-certified, Joint Commission-accredited, skilled nursing and rehabilitation facility. Our dedicated team provides outstanding short-term rehabilitation and subacute care. We are the only facility in the area that has full-time onsite Physician/Nurse Practitioner teams.These teams help prevent patients from requiring re-hospitalization. Our dedication is to uplift the lives of our residents, regardless of whether they require short-term rehabilitation or traditional long-term care. At Ledgewood, we understand that meticulous attention to detail is paramount, investing equal amounts of time and energy into our environment and the care we administer. Our mission is to act as a conduit between inpatient care and home for our short-term residents, while striving to attain and sustain the utmost level of functionality for each of our long-term residents.
Our Rehabilitation Department
Our nursing team provides care to a number of patients with complex medical needs. Our Rehabilitation Department consists of twenty full- and part-time therapists working in our rehab gym so that we can get you or your loved one home at the highest level of function possible. Our Rehab department is complemented by a rounding physiatrist focused on physical rehabilitation, pain management, and expediting your successful discharge back home.

Ledgewood is accredited by The Joint Commission. The center is also Medicaid/Medicare Certified and accepts coverage from many private insurers.
