Social services, counseling and discharge planning are important elements of the care and services Colonial Rehabilitation & Nursing Center provides. Social workers help to identify a residents psycho-social, mental and emotional needs and then develop individualized plans to meet those needs. Discharge planners define additional services and follow-up resources necessary to ensure a smooth transition from the nursing center to home.

At Colonial our outstanding short-term rehabilitation programs offer physical, occupational, and speech-language services. Our skilled therapists have expertise in both therapy evaluation and treatment. Services are designed to transition residents from post-acute care to home while maximizing quality of life. Our dedicated team of rehabilitation therapists, nurses, social workers, dieticians and support staff works with each patient and their family to develop a customized care plan designed to meet the patient’s individual needs.
Additionally, Colonial offers three non-invasive and medication-free therapeutic programs through a clinical collaboration with Accelerated Care Plus (ACP). The programs use electrotherapy, ultrasound and shortwave diathermy.
