The recently renovated Bay Path Rehab & Nursing Center in Duxbury is a 120-bed skilled center offering short-term rehabilitation and long-term care, in the Beautiful Coastal Town of Duxbury.
Bay Path specializes in cases that are medically complex. The center is accredited by The Joint Commission and is Medicaid/Medicare Certified. The staff is committed to providing high quality medical, nursing and rehabilitative care. Our programs are designed to respect dignity and individuality, allowing every resident to experience the best quality of life while maintaining or regaining function, health and well-being.

Our Rehabilitation Department
Our nursing team provides care to a number of patients with complex medical needs. Our Rehabilitation Department consists of over fifteen full- and part-time therapists working in our rehab gym so that we can get you or your loved one home at the highest level of function possible.

Our Rehab department is complemented by a rounding physiatrist focused on physical rehabilitation, pain management, and expediting your successful discharge back home.

Programs/Services
Bay Path offers a wide range of amenities and services including a Barber Shop/Salon, musical entertainment, and onsite activities.
Social services counseling and discharge planning are important elements of the care and services Bay Path Rehabilitation & Nursing Center provides. Social workers help to identify a resident’s psychosocial, mental and emotional needs and then develop individualized plans to meet those needs. Discharge planners define additional services and follow-up resources necessary to ensure a smooth transition from the nursing center home.
